The best-evidenced predictor of safe care is **nurse hours per resident per day** — hands-on nursing time per resident, measured by CMS from payroll. About **4.1 total nurse hours/resident/day** is a staffing-adequacy reference (not a pass/fail line).

In California, the median home reports **4.23 hours/resident/day**, and **656 of 1,039** meet the 4.1-hour reference. A high overall star does not guarantee high staffing.
