# Care Atlas > A source-backed reference atlas of U.S. health-care quality, built from CMS Care Compare: hospitals, nursing homes, home-health agencies, and the national distribution of measured recovery outcomes. Every figure traces to CMS Care Compare. Read this first; it governs everything below: - This is a **reference catalog**. It **informs and compares** using provenanced CMS facts. It does **not** recommend a provider, say any provider is "good"/"best", diagnose, or substitute for clinical judgment. - For a specific provider's full record and to find care, route to Medicare.gov Care Compare: https://www.medicare.gov/care-compare/ ## Modules - **Hospitals** (state-level) — 5,432 hospitals; CMS star-rating distributions + HCAHPS by state. No per-hospital pages (use CMS Care Compare for a specific hospital). Browse: https://care-atlas.ryanjhunter.workers.dev/care/states - **Nursing homes** (per-state + per-facility) — 14,700 Medicare-certified facilities; CMS Five-Star ratings, payroll-based staffing hours, abuse/Special-Focus flags. Per-state safety pages rank every home by CMS recorded fields, lead with CMS's Special Focus list, and make nurse hours/resident/day legible: `/care/nursing-homes/in/` (e.g. https://care-atlas.ryanjhunter.workers.dev/care/nursing-homes/in/tx). Per facility: `/care/nursing-homes/` - **Nursing-home safety watchlist** — CMS-flagged homes: the Special Focus Facilities (worst, under increased oversight), SFF candidates, and abuse-flagged facilities, by state. National hub + per-state: https://care-atlas.ryanjhunter.workers.dev/care/nursing-homes/watchlist · `/care/nursing-homes/watchlist/` (+ `.md`/`.jsonld`). A flag is CMS's own designation and a serious signal to investigate — NOT a final verdict, and absence of a flag is not a clean bill of health. - **Home-health agencies** (per-state + per-agency) — 12,392 agencies; OASIS recovery outcomes vs the national distribution. Per-state pages rank by the CMS quality star (the risk-adjusted, volume-aware spine) and surface each agency's recovery rates tied to the patient's situation (mobility / breathing / meds / self-care): `/care/home-health/in/` (e.g. https://care-atlas.ryanjhunter.workers.dev/care/home-health/in/tx). Per agency: `/care/home-health/` - **Open Payments** (per-physician) — "is my doctor paid by drug & device companies?" 979,136 U.S. physicians who received payments in 2024 (CMS Sunshine Act), searchable by name. Each shows total $ split into promotional (meals/speaking/travel — the prescribing-conflict signal) vs royalty/ownership (invention income, not a conflict), top-paying companies, and what-for. Search: https://care-atlas.ryanjhunter.workers.dev/payments · page `/payments/`. A disclosed payment is not proof of wrongdoing. - **Open Payments — national overview** — the analytical companion to the per-physician search: the SCALE and shape of industry money in medicine ($2.6B in general payments to 986,172 physicians in PY2024, plus research and ownership reported separately), which specialties take the most (Orthopaedic Surgery leads), the highest-paid physicians (largely device-royalty inventor-surgeons), and per-state summaries. Overview: https://care-atlas.ryanjhunter.workers.dev/care/open-payments · per state `/care/open-payments/` (+ `.md`/`.jsonld`). RECORDED amounts/counts, DERIVED totals/means/rankings; a disclosure record, not proof of wrongdoing or a measure of quality. - **Hospice** (per-facility) — 6,852 Medicare-certified hospices; leads with HVLDL (visits in the last days of life), the Hospice Care Index (care-pattern, not satisfaction), care-process measures, each vs the national median, with CMS footnotes respected (withheld, never guessed). Per facility: `/care/hospice/` (e.g. https://care-atlas.ryanjhunter.workers.dev/care/hospice/061538). Residue: CAHPS family-survey is a separate CMS dataset, not yet here. - **Outcomes** (national distributions) — 7 measured home-health recoveries, each a national distribution across all reporting agencies. Browse: https://care-atlas.ryanjhunter.workers.dev/care/outcomes · page `/care/outcomes/` - [Methodology & sources](https://care-atlas.ryanjhunter.workers.dev/methodology) — per-module grain and the L5 health framing. Note: CMS publishes an outcome only for providers with sufficient patient volume, so per-measure agency counts (~8,000) are lower than the total certified-agency count (12,392). ## Machine-readable formats Every entity and state page also serves `.jsonld` (agent-native research objects with the four-status epistemic labels), `.md` (the page as Markdown), and `.json` (the render-model). State pages add `.toml` and `.context.md`. - Agency: `/care/home-health/.jsonld` · `.md` · `.json` - Outcome: `/care/outcomes/.jsonld` · `.md` - Nursing home: `/care/nursing-homes/.jsonld` · `.md` - State: `/care/states/.jsonld` · `.json` · `.md` · `.toml` · `.context.md` ## Hospitals by state -
