- RECORDED — each recovery rate is a CMS-published, risk-adjusted OASIS outcome (the share of patients who improved), banded by national quartile (top 25% / typical / bottom 25%). A rate of 99%+ is flagged unsized, never rewarded, because CMS publishes no patient count to size it.
- NOT HERE — this dataset has no hospital-readmission or ER-visit rate (the other half of the home-health story); check that directly at Care Compare.
