Nursing homes CMS has flagged

CMS names a small set of nursing homes as Special Focus Facilities — the worst persistently-low performers, placed under increased federal oversight. Nationwide: 88 SFFs, 440 candidates, and 1482 homes with an active abuse citation.

A flag is CMS's own designation and a serious signal to investigate — not a final verdict (a home can improve off the list), and the absence of a flag is not a clean bill of health. Verify current status at CMS Care Compare.
