A Special Focus Facility (SFF) is among the worst persistently-low performers, placed under increased federal oversight; an SFF candidate is next in line; the abuse icon means CMS cited the facility for abuse, neglect, or exploitation where a resident was harmed or potentially harmed. A flag is a serious signal to investigate and ask hard questions — not a final verdict, and a facility can improve off the list.
